About Brooklyn Senior Care
A facts-only guide to Brooklyn's nursing homes and the home health agencies that serve Brooklyn, built from the federal government's own data. It doesn't recommend providers, rank them or give medical, legal or financial advice.
Where the facts come from
- Nursing homes: every row in CMS's Provider Information file with state NY and county Kings (40 homes), plus every row in CMS's Health Deficiencies (670 citations) and Penalties (28 entries) files for those homes. CMS dataset dates: modified September 30, 2026, released October 8, 2026; CMS's processing date in the rows is September 1, 2026.
- Home health: CMS's Home Health Care - Zip Codes file, filtered to Brooklyn ZIPs, and the matching rows of its Home Health Care Agencies file (30 agencies), released July 15, 2026. CMS lists its next home health update as October 21, 2026 and its next nursing home update as October 28, 2026.
- Brooklyn ZIP codes: the US Census Bureau's 2020 ZCTA-to-county file. We use the 38 ZIP code areas whose land lies entirely in Kings County, and leave out 11416 and 11417, which only touch it.
- Explanations: CMS's Five-Star Quality Rating System page, its Technical Users' Guide (September 2026), its data dictionaries, and Medicare.gov's coverage pages.
How we check it
A script refetches every source before each build. Every quote must still appear word for word on its page, every cell of CMS's scope and severity grid must still appear in the guide, and every nursing home row, citation, penalty, agency row and ZIP list must still equal what CMS's API returns. The site won't build if anything differs. Last check: October 14, 2026, 70 quotes and 72 CMS record checks.
What we calculate, and what we quote
- Quoted or copied from CMS: ratings, bed counts, staffing hours, turnover, citation descriptions, scope and severity letters, correction status, penalties, home health measures, footnotes and ZIP lists. We change capitalization of home health agency names and addresses for reading; each agency page also shows the name exactly as CMS prints it.
- Our arithmetic, labeled where it appears: total certified beds, share of beds in use, minutes of nursing time (CMS hours times 60), months since an inspection, counts by rating or letter, straight-line distances between CMS map coordinates, and the 2026 Part A calculator.
- Our words: the "Our reading" boxes. They restate figures from the same page in plain language and never compare a provider with another one.
Gaps and limits
- CMS's file cuts some nursing home names at 50 characters; those pages say so and show CMS's legal business name.
- A home health agency appears for a ZIP when CMS's file shows it served at least one patient there in CMS's data period, not because it advertises service there or has room for new patients.
- No prices: CMS doesn't publish what nursing homes or agencies charge. The calculator uses Medicare.gov's 2026 Part A amounts only.
- Assisted living and other residences that aren't Medicare or Medicaid nursing homes aren't in CMS's nursing home file, so they aren't here.
- Owner and manager names from CMS's ownership file are left out because many are private individuals; we show CMS's ownership type and chain name.
- New York's long-term care ombudsman site shows a bot check to scripts, and the eCFR resident-rights page returned nothing to our script, so we don't quote either. We don't work around blocks.
The dataset
brooklynseniorcare-2026.json is the frozen snapshot this site is built from (CMS rows, quotes, source URLs, query details). SHA-256: 6296aeadb2643ff2501ccd5d27caf45aa366a0ebc7cce7fd4649ae20ea32d08e. The nursing home CSV is our extract of it.

Corrections
If something here doesn't match CMS or Medicare.gov, tell us through North Coast AI with the source link. We correct against the official source. Providers who think CMS's data is wrong should take it up with CMS or New York State's survey agency; we can't change CMS's figures.
