Brooklyn Senior Care

Home health agencies serving Brooklyn, by ZIP

CMS lists 30 Medicare-certified home health agencies with at least one patient in a Brooklyn ZIP code. Pick your ZIP to see which ones CMS shows there.

Showing all 30 agencies.

What the ZIP list means, in CMS's words: “This file contains information on the areas served by the home health agency. It is based on OASIS assessments submitted by the HHA during the data collection period. There is one record for each ZIP code in which there was at least one patient served by the home health agency.” (CMS home health data dictionary) So a listing means the agency served someone there during CMS's data period; it isn't a promise it will take a new patient. Brooklyn ZIPs here are the 38 Census ZIP code areas whose land lies entirely in Kings County.

CMS home health data released July 15, 2026. Star rating: CMS quality of patient care, a numeric rating from 1 through 5, in increments of 0.5.

What Medicare covers at home

“Home health includes a wide range of health care services that you can get in your home for an illness or injury to help you get better, maintain your current condition or level of function, or slow your rate of decline.” (Medicare.gov)

To qualify you must be homebound, which Medicare.gov explains as: “Leaving your home isn't recommended because of your condition or you have trouble leaving your home without help (like using a cane, wheelchair, walker, or crutches; special transportation; or help from another person) because of an illness or injury.” (Medicare.gov) and “You’re normally unable to leave your home and leaving takes a lot of effort.” (Medicare.gov)

“Before you can get home health services, a doctor or health care provider (like a nurse practitioner) must see you in person and confirm you need home health care.” (Medicare.gov) “They must also order your care, and a Medicare-certified home health agency must provide it.” (Medicare.gov) “If your provider decides you need home health care, they should give you a list of agencies that serve your area.” (Medicare.gov) “Your provider must tell you if their organization has a financial interest in any agency listed.” (Medicare.gov)

Medicare.gov lists what Medicare doesn't pay for, including: “24-hour-a-day care at your home”; “Home meal delivery”; “Homemaker services (like shopping and cleaning) unrelated to your care plan”; “Custodial or personal care that helps you with daily living activities (like bathing, dressing, or using the bathroom), when this is the only care you need” (Medicare.gov).

“You pay nothing for covered home health services.” (Medicare.gov) “After you meet the Part B deductible, you pay 20% of the Medicare-approved amount for Medicare-covered medical equipment.” (Medicare.gov) “In most cases, "part-time or intermittent" means you may be able to get skilled nursing care and home health aide services up to 8 hours a day (combined), for a maximum of 28 hours a week.” (Medicare.gov)
